Certainly Bill Marriott and Jon Huntsman Snr. (not listed above but still worth more than 1 billion) have received the SA.

Nobody that I am aware of. Chesney does not speak of it, neither does Hans Mattson.

That is for John to decide. Although he approached me to do the podcast, he declined publishing it after being threatened by his then stake president. Now he has a new SP and John is intimating that a disciplinary court is inevitable, he might as well publish our podcast on the Second Anointing, the BoM and other matters.
Maybe 50% just pay the club dues in order to continue access and influence with a network of groomed gullible people willing to invest or drop money into MLM / bogus investment ventures.
